How It Works

Earning reward points is simple, as you’ll earn reward points for every dollar you spend here at SURV in Forks Township. Here’s how our SURVing Up Rewards program works:

• Each time you dine, tell your server that you’re a SURVing Up Rewards member and provide them with your phone number. Your points should be automatically applied to your account, you can always ensure this by checking your Surv app to see your points.
• Spend a dollar, earn a reward point. Yes, it’s really that simple here at SURV.
• Earn double points for every dollar spent, Monday through Thursday, from 4-6 p.m. as part of our SURVing Up Double The Rewards Program.
• Provide us with your birthday at time of registration and keep your eyes open for a special birthday promotion coming your way!
• Points never expire, meaning you can save them as long as you like, and for the best unlockable perks!
• Track and manage your SURVing Up Rewards points and membership easily with our app, available on iOS and Android devices.
• To become a rewards club member, scan the QR code with your mobile device or simply click on the Sign Up button.  You can also sign up during your next visit to SURV by scanning the QR code located on your menu or by asking your server for more details.

As you earn reward points, you’ll be able to unlock exclusive perks only available to SURVing Up Rewards Members. Redeem your points in the My Rewards section of our app, or by speaking with your server at the beginning of the meal. Check out some of the perks you can earn:

• Get $15 off your check by cashing in 250 Rewards Points.
• A VIP dining experience with Chef Tim. Find out more about this unique dining experience by speaking with your server.
